I was a F105 crew chief strapping a pilot into his seat for the next launch.I do not rember his name but we watched the whole crash sequence from my reventment, me standing on the ladder and the pilot standing up in the cockpit screaming for the pilot to punch out.
As I remember the pilot was about half way down the runway in full afterburner when the engine made a strange burping / hesitation noise as the pilot started to pull up.The plane settled back to the runway and began disintergrating as it slid the rest of the way straight down and off the end of the runway.The firefighters / rescue foamed what was left. I heard later that the pilot was still breathing foam in and out of his mouth when they reached him and later died.
