I assembled a package that included my orders to Takhli, a narrative of what my duties were including the facts surrounding my drive twice per day along the perimeter fence to go from FM to DP to Base Supply and return and a statement that included my duties as an SP augumentee. I don't remember where I found it but I did have a map of the base that was included with my claim. I'm sure I found it on line while scouring the net for this information.
I provided all this to my County VSO and they assembled the package with the assistance of the American Legion.
I can't say enough good things about my VSO and all the assistance they provided. After more than two years of doctors reports regarding diabetes and related issues such as neuropathy, depression and several others attributed to diabetes, I just received notice of my total and permanent rating effective July 1, 2013. It is retroactive to that date.
Sorry I can't be of more assistance but the County VSO's did all the heavy lifting for me after I brought them my initial claim. I do know it helps to have your private physician complete the DBQ for Type II Diabetes as this form covers all the significant issues regarding Type II as well as all of the related issues. It also helps to have a specialist provide documentation regarding your condition (Endrocronlogist) (sorry about the spelling). This is the "pea-pea" doctor and is recognized by the VA as a better judge of your condition.
