VW Passat 2.0D 16v BKP 2006 - Plastic inlet manifold flaps #

Hi
Has anyone experienced the plastic inlet manifold flaps being sucked into engine?
Looking at one with 2 flaps missing and no compressions on 3
The engine has been over filled,could this have caused the problem or is their more to it?

Taken head off and found number 3 piston smashed and head damaged beyond repair, what a great piece of design
