Hi Sean

I'd think the ECU is able to set the code possibly based on the voltage level from the fan module to the ECU.
From your description I'd say the voltage comes from the fan module and is modified by the ECU as required. So if the voltage should be 8/9/10 volts and is only 4.2 then a code is set.

You could unplug the fan module and see what happens to the voltage (but the 4.2 might be coming from the engine ECU if the fan module is completely dead)
