A bad remap could, but you would need to be able to read the ecu to see how much extra boost has been requested and what the limiter has been set at, or drive the car to see what boost pressure it reaches and compare to standard model. Most tuners I know and me included only aim for about 150mb increase on standard units, some may push for 200 to 250.

Also I've seen cars after a turbo failure that hasn't had the intake and intercooler cleaned out properly causing the engine to over rev and turbo to overspin, causing damage.
