Yeeeaahh!!! that's exactly what confused me at first too! I saw a ratchet belt at a friend's house and at first I thought it was more marketing. But then he let me try it on and two days later I ordered the Ratchet Belt for Concealed Carry myself. The mechanism is sturdy, it doesn't come off at all. Even when you move actively, no problems. First off, you have to get used to the clicks, but then you get used to them. And, by the way, it fits much more precisely than regular holes. I won't go back to the regular ones.
