The protocol surrounding touching the Royal Family has historically been so strict that a simple breach like a hand on the back, even by a well-meaning dignitary, was enough to spark outrage.
Actually, the media were the ones who were outraged. The royal was usually not bothered at all and the thought the media was making a mountain out of a molehill because somebody took their arm or touched their back when guiding them.
Also, when William says they want to be addressed by their first names, rather than as Duke and Duchess of Cambridge, he does not mean "Call us Wills and Kate"! He wants them to be introduced and referred to as William and Catherine.
