Trevor Rees-Jones said that he jumped in the front seat at the last minute because he had seen Henri Paul drinking, the crowd of paparazzi outside was getting aggressive and Dodi was focused on getting to his apartment although there was no reason for them to go there when they were perfectly safe inside the hotel. He had worked for the al Fayeds and knew they didn't listen to anyone, they issued orders. Trevor was the only one who wore a seat belt and the only one who survived.
Diana may not have worn a seat belt because she was accustomed to sober, trained drivers and assumed Paul was the same. All reports say that he was not staggering or slurring his speech, so unless you were at the bar counting his drinks you wouldn't have known how much he had to drink. He was, apparently, a high-functioning alcoholic. The al Fayeds denied this strenuously, but it seems to have been a known fact among the employees. Add to that the Mercedes had been in an accident and although cosmetically repaired, was not safe and should have been junked. The car had been stolen, wrecked then rebuilt and put back on the road. The structure of the car was compromised, probably the braking and steering were compromised as well, and there was no controlling it once it roared into the tunnel with a drunk behind the wheel.
