If you're in France, I think that the salad goes last - at least that what I keep hearing. Maybe there are other breakdowns in more elaborate dinners, like breaking the entrees into fish, fowl, meat, etc. Is that what you had in mind? Here at my restaurant we sometimes combine the first 3 courses if there is a mix of "first courses" at the table (this prevents one or two people from eating alone and shortening the time for the delivery of the entrees). In really formal dining, you wouldn't do this. we try to keep them seperate, but sometimes it's in the best interest of the table (and the restaurant)to combine them.
