You have a choice of how you do the assessment in VT..you can either do a portfolio, a teacher's assessment (that is what we do) or standardized testing. Yes, there is a form for the teacher. I have another homeschooling mom who used to teach school come to my home in the spring, and she does the assessmment, mails it to me, and then I send it in.
