Hi-
We're new to homeschooling and are filling out the enrollment form. There is a form in there that asks for an educator or health care professional to provide a statement as to whether or not the child has a handicapping condiion that affects learning. Does this form have to be filled out? Is it required? If it is, what assessment tool have people used for young children?
