We instituted this in our building formally this past year. We love it, great results!! The really great thing is that we introduced it as a regular ed intervention (yes, that's right, no IEP for them). They have to be mild cases 1-2 sound errors or simple syntax errors. They need to show sufficient progress over time or we move to and IEP (which we have not needed to do yet.
We have forms for parents for:
Overview of program
A very quick screening results page with consent signature request (all on same form)
Data forms
Update forms (for every quarter)
As I stated already but worth mentioning again.... We LOVE it. As you can imagine it is a really time saver for the kids and whole teams, way more efficient use of time.
