Hi - It's kinda odd for a 4th grader to only have word and phrase reps. Usually stuttering at that age consists more of part-word reps with some blocks. You said "periodic" - how often does he do his repetitions? To encourage self-monitoring, you can play games where first he would catch your pseudo-stuttering and then exchange roles where you catch his fake-stuttering. The big thing with monitoring is to not make the consequence for stuttering a negative thing. Another thing to try is start a transfer hierarchy where you both come up with places in the school or situations in school that are harder speech wise for him. Then, start with the easiest and have him use one of his tools in each - rewarding him for either every accomplishment or every other one depending on the child. Transferring the tools outside of the speech room will also increase awareness and his self-monitoring by putting more responsibility on his shoulders.
