I would try to introduce th since it is a very common sound in speech. What are the substitutions for th? Also with the students I have worked with that have Apraxia it is often helpful to try a new sound bc it is sometimes surprising how well they do and it will help decrease boredom and a lack of motivation in therapy.
If that doesn't work I would go with the medial L since the child can do L in the initial position.
