How about using the Super Duper "Wh" questions cards. Model the question as you show the picture and have the chid repeat. Then gradually you will be able to let the child take over and show you the card as he asks the question by himself. I find that correcting this takes a lot of repetition. Choose one particular type of question to target and focus only on that. After some time, I then catch the child in spontaneous speech and repeat the question just as he saidit in a questioning voice to see if he can correct it. Example: "What that is?" CAn the child state the question in the correct manner whe he hears me say it? If not, I would say, "You mean, What is that?. I would put more stress on "is".
