It looks like you need to dig the hole about 4 times the size of the root ball and amend the soil with soil conditioner. They sell a great one a Lowes and home depot has some as well. Hold off on any fertilizer but try some root stimulator. It seems like the roots may be suffering from root suffocation from the hard clay soil. Use a pick if necessary to break up the ground. Make sure that the palm is getting adequate water, not too much. Poorly draining soil can be the death of so many palms including trachys. I have killed two trachys this way.
