I have had adult clients in this situation. I used a commenting goal. Use environmental sabbotage and try to force a comment. Before viewing the irregular occcurance, do lots of out loud wondering about "what if", and " Have you ever" .The fall back prompt is to model commenting. I have found that instituting commenting goals have an added benefit. Setting up the thing to be commented on involves other staff or is seen by other staff.I have connected with institution or agency staff that I would not have interacted with with as an out agency itinerant. I have made inroads to more consultations this way.
