I agree with you. It appears that her transitioning to different settings is very difficult with her. I would work on assisting her with transitions in the classroom so that she is better able to cope with her daily schedules. She's almost to Jr. High and that could be a disaster if she couldn't transition from subject/class appropriately. I would also see how she interacts with her classmates, that way you could also address her rude comments, as that will also not go over well in Jr. High.
