Anyone else help with this post? Can you leave the tongue depressor in there for a few sessions while you drill (or have you done that already) and then try removing it while the child is still biting the back teeth together? That would be the only thing I would know to try. I always have them keep their tongue behind the bottom teeth and close the teeth and I can usually get a good production that I can build on. Not sure if any of them had jaw issues.
I am sure others will have ideas.
