I had the same issue with a student. Not so much the small mouth but just could not get the tongue elevation. Tried all of my tricks just like you. We finally dismissed for lack of progress just this year as a 5th grader. Sometimes you just have to say "enough is enough" and let it go. He was tired of it, hated being pulled from class, and wasn't really seeing the point of coming. I was frustrated as well that things were not improving. I still check in with him from time to time just to see how things are going. No changes.
