I've had the most success just telling the child to open his mouth very wide like an alligator, and then having him imitate the /k/ sound (which I model kind of like a throat-clearing sound) without moving his mouth at all. We call it the alligator noise. I got that suggestion from this board, and it has worked better than anything else I've tried.
