I am not sure whether you are still checking this board, but here is a sample taken from a similar artic report I wrote:
"Based on the results of this evaluation, speech therapy is not felt to be warranted at this time. However, as XXX is able to produce some errored sounds easily when prompted, it may be beneficial for XXX's family to begin practicing these sounds at home. Specifically, the /s/ blend sound production can be practiced by developing a list of words that start with /s/ and another consonant (e.g., /sp/ as in “spell”, /st/ as in “stop”, /sw/ as in “sweep”). XXX's family can first say each word, placing emphasis on the /s/ sound (e.g., “ssssstop”), while tracing one finger (the “snake”) up XXX's arm to provide a visual cue. XXX can then be asked to repeat the word in the same way, while tracing his own finger up his arm. It is recommended that XXX's family continue to monitor his speech and then follow-up with a speech-language pathologist after two months' time."
