Of course this is informal:
For sustained attention in class, our Psych does a "time on task" observation. You select a time period, say 5 mins at a time, and time the amount of time he is "on task" (doing what the teacher expects/doing what the majority of peers are doing) and the amount of time he is "off task." Do this for several time periods and come up with an estimate of % of time on task VS off task. You can measure in a variety of situations: Group-classroom, small group, individual TX, PE class.
