Feel like a broken record, but
I have had good success using Cycles with Down's kids because they only have to practice words at the word level, but you can see some carryover to conversation. Not going to be perfect, but it really helps with intelligibility. I have seen that most Down's kids that speak are phonological. Parent involvement and a parent-teacher notebook to write down observations is crucial for it to help. I have seen progress with a 15 year old that I was ready to give up on. The parents disagreed and I agreed to try Cycles. Then when they got on board and we saw great progress.
Don't worry about tongue protrusion. It's part of the syndrome and you are not going to fix it. It also has to do with small manibles and mouths that cause this condition as well, not just the tongue, although it is enlarged. Just make sure breathing issues are handled (tonsils, adenoids) aren't making it worse.
