It is 60 days from the signed request to test form. If the request to test arises at an IEP where SLP is not already involved, then 60 days from that date since the parent requests it and it is formally entered in the team notes, and everyone sign the permission to accept the IEP. If you send home a form to be signed by parents, then it is 60 days from their dated signature. Always keep the dated signature page. Of course you can test more quickly and have the meeting or addendum before the 60 day deadline.
