The horn relay should be wired a little differently than a normal relay and may only have three wires. One is positive in from the truck electricl system, two is ground from the steering wheel and three is positive out to the horn. The coil in the relay is wired with positive as is one side of the contacts all the time. The horn button on the wheel is what gives the coil its ground, that causes the coil to close the contacts sending positive to the horn itself and the horn is grounded to the chassis.
