TAX EQUITY and FISCAL RESPONSIBILITY ACT (of 1982) requires an administrator of certain plans to provide for "backup" withholding on distributions.
Is your account set up as an annuity, an employer deferred compensation plan or pension?
Did you make any withdrawals?
