It could be a Jeremiah Junction, but I have some doubts. I love those ones and have quite a few, but haven't ever seen that particular one...but it still could be theirs!

This one was available as a Freebie, but I cannot seem to find a link to it. So they've probably made it for sale now or discontinued with that author, as she's not listed in the designers they carry (Lori Birmingham).
