Everyone in my district does lesson plans. We all pretty much use the same format. It is in a grid. One sheet per day. going from left to right... TIme, Names of client/clients, target behaviors(goals), outcome, homework assignment. It takes a couple minutes per day and I keep them in a binder. Hope this helps
