Whatever you do, make the type large and the text as brief as possible. Something that the teacher can put a check by. Maybe list the strategies, putting some blank spaces by each strategy. The teacher can write the date that she tried that particular strategy and maybe a code letter for the results observed from using that strategy, like modeled correct response/strategy, it worked, didn't work etc.
Go over each strategy with the teacher beforehand. Do this because sometimes teachers interpret our suggestion very differently from how we intended them. Also put Referred by: and put the teacher's name so that she can feel some responsibility for implimenting the strategies.
Check in with the teacher daily for the first week so she knows that you really mean for her to implement the strategies. Always thank her for her extra effort with this student. A little appreciation goes a long way.
