Could you type up a lesson plan for each kid with their name and all goals numbered and listed at the top? Then make a table with 3 columns; one for the date, one for the goal number you're working on, and a wider one for the data collection. You could even make a fourth column for percentages and figure those out quickly at the end of the session to make the child's progress easier to see. I keep all my lesson plans in a notebook and just flip back and forth or sometimes I use a clipboard.
