Would it help to color code the goals? By this I mean writing the goals on index cards or a master sheet and using colored dot stickers to distinguish each student's goals. Then on your lesson plan sheets, you could use the same kind of stickers to determine which goal was for which day. You could sit down at the start of a week and put the stickers in your grade book in a certain order -- before you do any lesson plans. Then you could quickly refer to the master list (or set of index cards) and find the student's goals.
