I simply printed out a Microsoft Word table with three blank columns: a small column for the date, a larger column for the lesson plan, and then a very big column for notes on the session. I use that large column for keeping data, writing impressions or problems -- just anything that is important for the day. I can usually fit about four or five days' entries onto one of those pages. I keep a large pile of these blank forms on hand, and I keep adding them to the file folders as I need new ones. I just store them loosely in the folders, new ones on top. As for the folders themselves, I have them coded by what day of the week the child comes in. The days are written right at the top edge so I can grab all of a day's folders quickly. I also number them by what order they come in on each day. So, for example, Johnny's folder might say Mon 9/Thurs 5, which means he is my ninth student on Mondays and my fifth student on Thursdays. Each morning, I can grab that day's folders, stack them in the order the students will come in, and I'm set to go. I keep the pile on my desk and turn each student's folder over as I get done with it.

I keep a separate file folder for each student, and store them in a file box. I staple a copy of the goal page from the student's IEP into one side of the file folder, so at any given time I can refresh my memory on what his goals and objectives are.
This plan works really well for me.
