I recently started to work at a private clinic. The ax procedure is a 30 minute phone call (intake)and then assessment. I have found that the clinic does not have a good intake form and I am struggling to obtain approrpiate information regarding the background info / parent concerns / current level of functioning within a specific amount of time. Does anyone have a form or at least suggestions as to what should be asked during intake to obtain an appropriate amount of info and then determine what assessment tool is most appropriate to use? Thanks.
