In our district we are able to see articulation, voice and fluency children using a General Education Plan, which is a one page form that states service, focus of service and includes parent and principal signatures. It is intended for short term therapy and is reviewed every 4 to 6 months. No progress reports are required. OT and PT use this form also. If a child is on an IEP for other services, therapy would be included on the IEP.
