I serve two schools & being organized is critical. I have a big notebook (2 1/2 or 3 inch) for each school. Inside the notebook I have those plastic folders (with tabs), enough for each group of kids that I see. They are labeled by time slots. In each groups folder I have my data sheet/IEP goal sheet/etc for each child (all stapled or paperclipped together). I keep the data sheets on top and just staple more to them as they fill up. I usually write what sounds the student is working on or language goal, right to the top of the data sheet. Example: If the child has multiple artic errors, I write them all at the top of the page... then work through them. This keeps me from constantly having to look back at their goal sheet or test info. I use the Entire world of R screener with all of my "r" students, and I keep that with the stapled papers.
My attendance sheet is kept in the front pocket of the folder along with my schedule for that school.
Therapy is "quick & dirty" most of the time. I copy a ton of seasonal appropriate open ended worksheets and keep them in the back of my big notebook. I use those most every session and just write the stim words we've used int he speech session for homework (for artic) or some other language skill on the paper (just cross out the other directions & write your own). We play a game (anything from UNO to kerplunk) and all take turns with 5 to 10 drills then a turn at the game. If I have extra time for planning or want to do something specific for a student.... I'll copy ALOT of material (like with /s/ for a lisp) and paperclip it to the data sheet for that child. So instead of getting a open ended sheet they get something specific.
I've used alot of systems, but the big giant notebook seems the most streamlined thing for me. It means I only have two notebooks (they stay at the schools as I have a dedicated space) and another with general caseload info (IEP dates, elig. dates, parent phone numbers, travel reimburs. form, etc) that I take from school to school.
