I worked in a clinic with kids with CL&P for over a year-- and there is much research out there to show that oral motor exercises for kids with clefts are not effective, especially if there is co-occurring VPI/hypernasality. (No amount of blowing exercises will remediate hypernasal resonance) That said, if they are producing glottal stops or pharyngeal fricatives for all pressure sounds (and I assume that they are), start with the /h/ phoneme to start shaping all of their productions. It helps eliminate the glottal stop because they are keeping their vocal folds apart by saying /h/. Have them start with /h/, extend the /h/ and then shape any pressure consonant from there.
