The paper is facing horizontally, and there are 3 columns on each page and about 10 rows(I have 2 pages for most of my kids because they have 4-6 goals). I copied and pasted one goal at the top of each column (from their IEP) and then I keep my data in the rows under the particular goal I am working on. I obviously can't address each goal every session, but when I'm planning I can tell at a quick glance what goals I need to address in future sessions. I have a working folder for each child where I keep the activities we are working on and their data sheets. I will print off new forms when I fill up this first batch. It was time consuming to set them up for each student, but it is the best system I have used so far.

I do keep data for just about each session. Sometimes it's tally marks for each trial, sometimes it's a list of vocabulary words that we worked on, sometimes it's just the date and activity. Writing something (regardless of how much and what type of info) helps me to know that I worked on that goal. In the past, I have had problems with neglecting the same goal over and over again and having very little data by the time progress notes rolled around.
