There are 10 lily pads and each student has a frog with their name on it. At the end of the session, they get to hop one lily pad. When they reach #10, they can take a prize from the treasure box. For most of my students, this means that they get a prize about once a month (depending on how often they come). I have thought about increasing the frequency of rewards by having 2 prize options...treasure box on #5 (and then move back to start) or keep going to #10 and earn "bring a friend" or "choose your favorite game." For my board I made the lily pads out of felt. I bought a package of frog cutouts at a teacher store, laminated them, wrote their names in marker, and put a dot of velcro on the back. I used a frog border and bought some fake catails and plants to decorate the bottom of the board. You could have various other themes...planets, stars, a trip through the zoo, baseball diamond, football field, dinosaurs, etc.

I used to use sticker charts. I printed the ones from preschoolprintables.com. They have 20 squares. For kids who came once a week, they earned a prize every 5th box (I highlighted them). For kids who came 2+ times a week, they earned a prize every 10th box (again, I highlighted the box so they would know).
