I am a big fan of Van Riper's approach for older students whether mild, moderate, or severe, as long as they are intermediate or advanced, which a secondary student will be.

You may want to make your own form specific to fluency. Maybe some questions like:
2. Does this student avoid speaking situations?
3. How does this student's speech affect social relationships with peers/adults?
4. Do you have any accommodations currently in place for this student?
5. Would you like some ideas for accommodations for this student?
2. Do you avoid any speaking situations because of your speech (e.g. raising your hand to ask a question, talking to kids in the cafeteria)?
* I would do this in the form of an interview and if you think it's safe to use the word "stutter" after those questions, then ask,
3. What do you think caused your stuttering?
4. What are some things you do to help yourself?
