I make a generic lesson plan for each day of the week and then just print them as I need them. When someone's speech time is changed, I just alter the plan that is saved in a word documment. It is not perfect but works for me. I am interested in seeing the way others are dealing with this difficult issue.

Every year I try a new method and it is so difficult because the kids are grouped and so when you try to document each student individually you have several different folders to write in and it takes so much time and the next group arrives and you are still trying to make notes etc... The best method for me is to to write a generic Monday lesson plan. It might look something like:
8:30-9:10 Sarah- R final position, Mike- 3 step directions, vocab, slow speech, Sue (comes from music) retelling, grammar.
All- turn taking, eye contact, question asking
