Is this a "drum" breech type barrel?
meaning; does the nipple screw into a round barrel like extention from the barrel?
If so, some times that drum will turn forward just a fraction of an inch and the hammer does not fall squarely on the nipple. Instead it will strike the rear of the nipple and not cause enough pressure to ignite the cap, it will however deformate the nipple itself. There is a fix.
