Re: Bassetlaw PCT Transforming Community Services Market Place – 28th September 2009
I am writing to invite you to the above event where the PCT will outline for you our commissioning intentions and key initiatives we plan to take forward in the next year.
During the market place there will be a facilitated discussion with PCT lead officers that will offer providers the opportunity to explore different models of delivery that could create the greatest improvement in the health outcomes prioritised by the PCT. There will be three separate facilitated discussions, these will be:
· The End of Life Pathway.
· Chronic Obstructive Pulmonary Disease.
· Falls Awareness and Prevention.
We will also set out for you the processes we will use to inform providers of opportunities in the future.
We are planning to hold this meeting on 28th September 2009 from 12.00 m.d. – 3.00 p.m. at Bawtry Hall in Bawtry, Near Doncaster, South Yorkshire. I would be grateful if you could let Christine Jones know if you would like to attend a facilitated discussion and please indicate which area(s) you would like to discuss so that we can arrange facilitators accordingly.
