Information Sharing Events 14th and 28th May 09
Information Sharing Events were held at Boughton Pumping Station, Ollerton and the Belfry Hotel in Nottingham. The events were held to raise awareness amongst Health and Social Care Staff, users and Carers of the implementation of Information Prescriptions for people with long term conditions such as Multiple Sclerosis, Motor Neurone Disease and Parkinson’s Disease for example.
The Information Prescription project now in its second year – has put in place a series of links and signposts to help people to find information about their health and care needs. This could include things such as information about their condition, what treatments might be available, benefits people could be entitled to and sources of advice and support including support groups that are to be found within Nottinghamshire. Information Prescriptions will also help people to get advice and support from others who have similar conditions.
The Event also gave other projects an opportunity to share their current and future developments in both Health and Social Care. These included – Expert Patients, Self Help Nottingham and Self Directed Support.
The Expert Patient Programme is a course for anyone who is living with a long term condition. The aim is to help people to have more control of their condition. This can include offering advice about managing symptoms, pain, or healthy eating for example. There are six sessions run by people who have long term conditions themselves. On the day we heard an inspirational account of how the expert patient programme has transformed the life of a lady who lives with a long term condition and who has since become a tutor for the course.
Self Help Nottingham is a unique charity providing support and development opportunities for self help groups and their members in and around Nottinghamshire. Self Help Nottingham offers an information enquiry service about local and national self help groups and networks including an annual Directory of Self Help Support Groups which is available to all GP’s and other primary care staff. They also offer practical services for self help support groups such as free and low cost meeting rooms, help with posters and publications, newsletters and mailbox/redirection for groups. The Presentation explained how Self Help Nottingham can offer support and advice to those living with Long Term Conditions and their families and carers to set up or find an appropriate support group. They also deliver a free training programme each year for groups on issues such as funding, managing meetings, welcoming new members etc. A new range of training is also being offered to health and social care workers engaged in supporting self help groups locally.
A presentation was given about the changes to Social Care currently taking place in Nottinghamshire Adult Social Care and Health. This change is called Self Directed Support. Self Directed support will provide those who are eligible for social care to have a personal budget. A personal budget will give service uses more choice and control and flexibility over how they wish to arrange their support.
Examples were given to show how a personal budget could be used to help a person with a long term condition live the life they choose.
The afternoon sessions gave everyone an opportunity to talk to the individual presenters and ask questions about their services. Discussion was lively and informative and feedback was collected and further information about all services has been sent to those who requested it.
The next steps for Information Prescriptions will be to increase the numbers of people e.g. District Nurses and Social Workers able to offer Information Prescriptions – for example it is envisaged that when attending health clinics or having a social care assessment, patients/service users will be offered a prescription related to their long term condition.
