I did as you suggested and still does not work. BTW - I also tried to hook the ringer in parallel to the phone with a 3-way connector and I get the same result - the external ringer sounds on an incoming outside call, but not on an intercom or doorbell call. Almost seems like the ring trigger voltage is lower on system calls than on calls from CO. Is this the case?
