I have the same issue. In fact, I have scheduled a "vendor meeting" to meet with the phone company to try to figure out the problem. I thought it was their channel-bank, but now that you describe the EXACT same problem I am having, I'm not so sure.
See if when it happens if the outside caller hears a "beeping" noise. That's what they claim in my example.
Is there a channel-bank involved at your site?
