FWD to Outside Line

I am trying to setup an extension to forward to an outside phone number on a KX-TD1232 PBX and TVS125 VPS.

I have setup forwarding on the extension to forward to CO. When I call the extension from another one it just keeps ringing and ringing.

If I transfer an outside call to it the system recall timer picks it up and sends the call back to the extension it was transferred from.

If I dial the extension from within VPS prompts it tells me Incorrect Entry, and begins with the main prompt again.

I have the FWD to Outside Line enabled on the COS being used for the extension, but it just won't transfer for me.

Any ideas on what other settings must be set?
