Terminal Cancer. Although, my guess, if you have murdered a whole bunch of people, and are already the walking dead, what is a few more?
My husband worked with a woman who was a former flight attendant. It was her flight crew that got killed. Although she was no longer working for the airline, quitting less than 6 months before the bombing, she had to go to therapy because of it. As I am sure the families of the victims did as well.
It is hard to have compassion for a dying man who is a mass murderer.
