Always use the formal salutation unless (and absolutely unless!) you are sure that the individual is such a good friend of your organization that they prefer the first name basis. I always think it makes a nice statement if the CEO, when signing a personal letter, will line out the "Dear Dr. Jones" salutation and ink in "Dear Diana"... of course, with an appeal letter you can't do that unless you do a special run for certain individuals ... and hopefully your HNW donors aren't getting direct mail appeals!
