    Aurelia, Jamie, Catherine:

    I have always followed the protocol of using their full name and title, i.e. Mrs. Susan Smith, or Mr. John Green. If they hold a doctorate, then I use the full John Green, Ph.D., Program Director.

    When imploring the salutation line of a letter, I would then use:

    Dear Dr. Green;

    If their title (Mr./Mrs./Dr./Rev)is not provided, then I stick with their full name (Susan Smith) and leave it at that. I would never just use a "Dear Susan" on introduction.

    Once I have established a series of contacts, and they begin using their first name (especially in emails), then I will follow their lead on how to address them.

    As a reference point, Junk mail comes with my first name as a contact, to make me respond personally. The people to whom we are writing are not allowed to respond in a solely personal manner...they represent a foundation or corporate entity and respond as such, within the parameters of their role.

    My mother taught me that respect, formality and manners are never out of place, and you never get a second chance to make a first impression!
